## Health Information
The health and safety of every child in our care is our top priority. Please review these policies carefully.
### Enrollment Health Requirements
Before your child's first day, we require:
- **Up-to-date immunization records** (required by Minnesota state law). Children ages 3–5 must have the HIB vaccination on file before attending.
- **Completed health care summary form** signed by your child's physician, submitted within 30 days of enrollment.
Children arriving without completed immunization records on file cannot stay for the day.
### When to Keep Your Child Home
Please keep your child home if they have any of the following:
- Fever of 100.4°F or higher
- Vomiting twice in a day
- Three or more loose stools
- Unexplained fatigue or lethargy
- Eye or ear discharge
- Difficulty breathing
- Undiagnosed rash
- Lice or ringworm
- Chickenpox (until all lesions are crusted over)
- Bacterial infections such as strep throat — children must have at least 24 hours of antibiotic treatment before returning
### If Your Child Becomes Ill at School
If a child shows signs of illness during the day, staff will contact you immediately to arrange pickup. A quiet rest area will be provided until you arrive.
### Reporting Contagious Illness
Please notify the center if your child will be absent, and **report contagious illnesses within 24 hours**. We will notify families when a contagious illness is confirmed in their child's classroom.
### Emergency Authorization
Your enrollment forms authorize CDLC staff to take necessary medical emergency steps during program activities.
